I've been digging around on this and I've found that email readers
don't support the <form> tag ...

That's what I suspected. And that's the reason I avoided using the word "form"
in my previous response. On the other hand, most email readers will properly
format hyper links, which may launch a browser and direct users to a Web site.
And many email readers will properly format HTML "documents", which may look
just like a PDF document. I'm not suggesting any more than an HTML formatted
email for email readers that support that, and a hyper link to a proper HTML
form, for those who will fill in the blanks.
