I had a S/38 thermal lock once when the air conditioning failed in
mid-August.   Locked up tighter than a drum. Had to do the old 3-4 to get
it restarted. That's about the only time.

Not to mention, Charles:
In the 21 years that I have worked with System/38, System/36, AS/400,
iSeries, and i5 machines, I have _never_ seen a single one of them shut
down
and/or reboot themselves (without an operator first issuing PWRDWNSYS)
because of OS problems...

Of course, in one shop when the DP manager inadvertently unplugged the
system, we had a unexpected shutdown

(One of Murphy's laws is:  "It Works Better If You Plug It In")

And, in another shop, a new VP who came on a tour of the computer room
inadvertently pressed the EPO (which was next to the computer room door),
thinking it was a door release, again unexpectedly causing the system to
shut down.
