I'm guessing the PDF you mention is the HOWTO. AFAIK there is no way to specify a macro file on startup. But do you mean a macro file or a macrowithin that file to run on startup? tn5250 uses ~/.tn5250macros for all its macros. Do you want to use a different file? Or do you want to specify a particular macro within .tn5250macros to be run?
