This has happened to me several times.

I _think_ it may be because different servers are used to create new
source members and copy the data to them from the workspace. If the "copy"
one times out after the source member is created on the host RDi does not
appear to recognize this and subsequently treats the file as if it were
just local to the workspace. IBM have been made aware of these types of
timing issues and its is being looked at.

The "fix" you applied i.e. create new source member and copy/paste the
editor content into it is the approach I use.
