Thanks to Process Explorer I've determined the problem to be:
TSC.EXE - Trend Micro Damage Cleanup Engine.

While that process is running during startup my PC is basically dead. Time
to go get that cup of coffee, read a little and then get to work. Kills
20 minutes of productivity every morning but what the hey.
