Thanks to all for the ideas. We do use Qualtrics for many surveys and we build an export from our DB2 data to .csv and manually upload that file to initiate that survey process. Works well but this survey has some additional complications. We want to survey every student that cancels their application for a particular semester, and do that within a few days of when they cancel. So today there may be 1 for Fall and none for Spring, tomorrow 3 for Spring and none for Fall, next day none at all, next day 5 for Fall and 3 for Spring, etc. But we also need all for a semester in the same survey pool so the responses are all in the same database for analysis. So the option to do a one-time build of an export file for manual upload does not work. Our research office is also adamant that the survey has to be an invitation only survey that only those who get the email can answer. Qualtrics does an a web API that is very powerful but also very, very (did I say very) complicated to do what we need to do. I know we have the programming talent to build a fully custom application to send the emails that might be needed every day, generate a unique identifier so we know who is responding and write the responses to a file for analysis but wanted to look at what already exist first.

Keep in mind that with the addition of the Open Source languages, your options get fairly wide, and with the ZendDBi (MySQL version at this point) you can even use DB2 as the storage. We can argue "Native" if we really need to but I'm guessing you have a wide range of options with PHP, Python, and Node.js. Point is it will very likely run on POWER with IBM i. A search on "PHP based Survey System" yielded over 4 million hits with the first two pages showing many options that included open source download.

Does anyone know of a web survey package that runs native on IBM i? Something like Qualtrics or SurveyMonkey but just needs to do the response collection and not all the reporting these other tools do. We need something that could load the list of survey recipients from a DB2 table, send an email with a link to the survey, collect responses and store in DB2, send follow up emails to non-responders after a period of time.

I found this from IBM https://www.ibm.com/support/knowledgecenter/en/ssw_i5_54/rzau5/surv.htm but a little concerned about verion 1.0
