I played with Lime a while back and it seemed a useful tool https://sourceforge.net/projects/limesurvey

There's also UCCASS which also looks good.

Either of these when hooked up with the DB2 storage engine should work well.

I do note that this is an arena in which the Open source activity has died down a lot in recent years. I have to believe this is because there are so many free/cheap survey systems (Survey Monkey for example) out there (Survey Monkey for example), that for most people hosting their own is just not worth the effort.

Can't help wondering if the OP wouldn't be better off using a service. What I've done in the past is use SQL/RPG/whatever against my IBM i data and produce an XML or CSV from the results. That is then uploaded to to the survey system to send out the invites - or if the survey system does not offer that on the free account then I send the invites direct from the iBM i.
