If the FDF has all of the fields in the CSV, try using CA to create the file from the csv file and specify the FDF that you created.
I have tried this also. I tried using the FDF that was created with the Excel transfer. It gave me an error saying I couldn't use that FDF. I downloaded the file from the 400 to create the FDF file. When I tried uploading the file I get an error saying the AS/400 Field Reference file name is not correct.
