It sounds as though the problem is that the "Original LIC CD" was used to perform the LIC & OS install; i.e. that the D-mode install from the Save option(21) was not done. On some levels of v4r5m0 media that would have left a mismatch between some function in LIC and the Query Engine; i.e. the PTF MF26991 is missing, but SF66530 is applied. But from having installed to ISD [IBM Software Delivery; among its myriad names] level of LIC [probably resave level RE00304 with some additional PTFs; MF26991 comes only with TL01296., to get the system back up to a PTF level closer to or where it was before the disk rework, the cumulative and PTF package should be applied [i.e. not just the noted-as-missing PTF], or the LIC & OS should be installed from a D-Mode IPL.
