I asked the developers this whilst in Rochester, and was told the following:

Use the original Client Access V5R1 install CD, and select the "Additional
information and tools" menu option, and then Create a custom Client Access
Installation image.  This image is then created in a folder you specify.

To now integrate the service pack, you need to go the IBM Client Access
service pack FTP site, and download the PTFFORM.EXE file.  Extract the
contents, then overwrite the files in your installation directory with the
new files from PTFFORM. (You can't use the image from your iSeries - it
doesn't work.

Now, when the installation is done from your directory (burnt to a CD), it
installs with the latest service pack.  If you want to create a silent
installation, that's another story - check out the Client Access Express
V4R5M0 redbook for help with that.

This doesn't work - you need to use PTFFORM.EXE as detailed above.
