I upgraded our 620 from V4R3M0 to V4R5M0 the other weekend and the upgrade started to look for the OS on the tape devices TAP01 and TAP02 before accessing the cd. I would suggest that you vary off any tape devices before you do the upgrade. Once I did that the upgrade worked fine.
