There was a big write up about it in the Wall Street Journal last week.  Didn't read all the details, but as I remember, they had put in  a new system back in July that was supposed to control manufacturing, inventory, and distribution.  It seems that things have gone "haywire" since then and was allowed to continue to degrade over time!  If you want more info, I will try to find the write up for you.
