Inserting a record

Any ideas on a solution to this situation?

I have a file of about 20,000 items.  I wish to have comments atached to
each item. There might be zero to 500 comments for each item.  These
comments will generally be in order by the order file and the date the
comment was created.  I say "generally in order by date" because I need
the ability to insert additional comments immediately after an existing
comment grouping.

In other words, I'd like to do something like threaded e-mail.

One problem though has to do with updating the thread while someone else
is looking or updating the thread at the same time.
