Looks pretty much that my guess is correct; the conversion tool is throwing 
warnings about the heading "Short forms" which is at heading level 4 and the 
fact that there has been no definition for heading level 3 - if you look at the 
source for the page only heading levels 2 and 4 are defined. This warning is 
just stating a temporary heading level 3 was created as a defined one couldn't 
be found.
