You can use overlays.  Most printers support this and although it is a bit
heavier on the AS/400 cpu side, it is neat and easy and does not require
any PC stuff or special products.  This solves the printing side.  You can
then configure everything yourself to e-mail stuff like this, or buy
something like Gumbo spoolmail (www.gumbo.com).  With this you can email
your spoolfiles including the AFP background as PDF file for example.

We are looking for a solution which will allow us to laser print, Email, or
fax mapics forms.  Initially, we would like to use laser printers.    Any
