I'm not sure if this is the cause of your problem, but I see two things right 
off the bat.

First, in your script code, the spelling of the reference to your form element, 
"PRICEING", does not mathc the spelling of the reference within that element, 
"PRICING".

Additionally, there is no attribute called "name" within a FORM element. I 
believe the attibute you want is "id".

Hope this helps, but I'll keep looking.
