Like anything else, it's a business decision. If you have an intranet application with 100 users, you absolutely don't need load balancing or clustering. Failover, sure, but that can be as simple as a second hot spare and a NAT server.

I've got clients who have load balancing and automated failover, and clients who don't. True failover (including transfer of sessions in progress) is not simple nor is it inexpensive. If you don't need it, don't do it.
