I've only ever been aware of 'C' - cleared and blank. In fact searching our database that is all I see (over 5 years data). Not sure what 'O' is but you may want to scan your source for CIND15 (shouldn't be too many occurrences) and see what if anything is adding this value. On looking at PLP15 (AP Transactions), there is a field called "Cleared item indicator". The file contains three values blank, "C", and "O".
