Just thinking quickly here.
1) you will need a Payment Method of EFT or similar so that you can hold the
credit card number against the customer.
2) You want the order to be suspended until the EFT transaction has been
approved by the bank (you don't want to despatch without an authorisation)
3) So you will want to extract suspended orders where the customer has a
default payment method or a type relating back to EFT.  The authorisation
would release the order.
