I work for a company that is publicly held so, Yes we do have a code review.

Programmers are not allowed to make any changes to the production environments.

We have a process, which is automated, by which work is assigned to a programmer.
Once the desired changes, improvements, etc. are completed by the programmer the next step is for one of the
other programmers to review this code and the user/users to approve the testing (not to imply the programmer
has not tested also). Once both of these steps are completed a request to move the changes to production can
be made. When the request is approved, (weekly unless an emergency) the changes will be moved to the proper
production environment by the operations staff. We use a change management software for that process.
The production environments do not contain any source code so only the compiled objects are moved.

And yes, we are audited annually on this process to insure compliance with our standards.
