Two comments... no.. three.

1) ask your users which report they already like and use that look.  Add
missing fields to your taste. 
2) use PRTF.  It'll save you a lot of grief and add tremendously to
consistency
3) consider using Figlet for an  ascii art company heading. see an example

I'm creating the first of several reports and would like for them to be
consistent. In the "O" specs I want to put program name, report title, page
number, date, and time. I've seen these things laid out in a variety of
orders for lots of different reasons. Thoughts on what should go where and
why? TIA.
