It's nothing more than old habit, but I always put the program name left
adjusted on line 1 with the user and timestamp on line 2.  I center the
Report Title on line 1 with additional detail underneath on line 2 if
necessary, and the page number right adjusted on line 1.

Like so many other style points, I think the key is consistency.
