Turns out a RPG program had shared/update exclusive lock on the file. Had to end that job in order to add permissions.

Your failure to use authorization lists would cause this.

There should be two users authorized to the file:
- The owner
- *PUBLIC and *PUBLIC should have *AUTL.

You can add/change/delete users to an authorization list while the object is in use.
