IPDS printing "off"

Hi Folks,

 

We have an IBM InfoPrint 40 laser printer and it had some problems with feed
mechanisms and rollers last week and IBM fixed all of that. The primary use
of this printer is Invoices and Statements. Things were fine at first and
then we noticed the print (on preprinted forms) was off starting at the far
left AND from top to bottom.

 

I have tried turning off the printer,  varying if off and on, etc. to get it
to "reset" with no luck.

 

Does anyone have any idea what this might be or how to reset it ?
