Problem has been resolved! The credit goes to Jim Franz, who suggested that there might have been a problem with the device name on the console. Our machine had QDEVNAMING set to *S36, which resulted in a console name of "W1". For some reason, the V4R5 installation process does not like this at all. I powered off the console, changed QDEVNAMING to *NORMAL, deleted device description "W1", and turned the console back on. Auto-configuration assigned the name "DSP01" to the console. I ran the installation procedure again, and everything is proceeding smoothly now. Thank's to everyone who offered suggestions.
