I am very interested in this topic. For what it's worth, I consider this to be the single greatest failing of AS/400-centric development. As a community, it seems to me that we cannot forget our "little development shop" environments and we either won't or don't know how to embrace proper procedures. We also suffer because we frequently don't include proper testing time, staff, or equipment in the implementation estimate. I have some ideas and, like many on this list, quite a bit of practical experience - both formal and informal. In the event that the "found" material isn't sufficient, I would be interested in helping to create checklists, design documents, and tools.
