   We handle the information and telecommunication systems for several
related companies and my standing mandate for switchboard operators is NEVER
to transfer any call to ANY extension that they are not personally familiar
with.  Have been bit by this bug before!  This is something to check out in
your voice mail systems to be sure they do not allow blind transfers to
non-valid extensions. 90, 900 (9=outside line, 0=local operator, 0=long
distance operator), as well as any programming codes your switch may use to
allow remote administration ("Please transfer me to *99", get modem tone,
make changes to allow remote access, IT HAPPENS FOLKS!)  And this works on
MOST PBX's that allow trunk-to-trunk transfer as many of the newer switches
do.
