Addressing for a 5394 controller is as follows:


        Twinax
Port \  Address     0   1   2   3   4   5   6

  0                00  01  02  03  04  05  06
  1                07  08  09  0A  0B  0C  0D
  2                0E  0F  10  11  12  13  14
  3                15  16  17  18  19  1A  1B   <---  only on a 5294


Your example, Address 09 on Port 1 is switch setting 02 on the device.
