Was it a possible a keying error? That someone did not use the field exit key? Basically, the shipped quantity was multiplied by a factor of 1000 i.e. if the original qty was 2, the shipped qty was 2000. This problem has caused a major misstatement in our month-to-date financials to the tune of tens of millions of dollars. Fixing this problem is one thing and figuring out why this happened is another thing. We may do journal entries to correct the situation, but any thoughts on why this problem may have occurred.
