On our earlier version of BPCS (405 CD) we have had other problems related
to your mess.

The General Ledger only allows up to 9999 journals per fiscal month per
journal prefix. We have gone over that ceiling.

In our case this was triggered by someone altering journal rules, without
understanding all of BPCS, at a time when all inventory flowed into IN
journal. We now have most inventory flow into journals where the prefixes
are similar to the inventory transaction types, and some are tracked in more
detail than others.

FMA is the shop order file for materials from MBM. We have had duplicate
FMA records for some orders where the MBM was A-Ok.

We never did figure out what was causing it. When we see an order like
that, we close it, launch a replacement.

Before we got raid, we had hard disk crashes which corrupted some files. We
found out about the corruption when a backup failed.
