Repeated notes

We are under an AS/400, with BPCS 6.04.

We are experiencing some problems when loading notes in a customer order with
ORD700. Sometimes the notes we add appear then repeated from 1 to 9999 times,
in customer orders related documents.
We do not know how to avoid this because it is annoying to remove the notes
one by one.
Has anyone gone through this? Any ideas?
