Davide and I solved this offline but never posted the solution here.

Root cause was that permissions weren’t set properly for the PHP installation directories.

To solve the problem, run these two commands:

CHGAUT OBJ('/QOpenSys/pkgs') USER(*PUBLIC) DTAAUT(*RX)
CHGAUT OBJ('/QOpenSys/pkgs/lib') USER(*PUBLIC) DTAAUT(*RX)
