Ok.  Use shading judiciously for grouping elements.

In addition to inquiry, a maintenance program needs to assist with the
addition, modification, and deletion of records.  Are there any
recommendations in that regard?

One design pattern might be using an "option" dropdown box or input box next
to the records in question.  Another idea is displaying a popup menu as the
mouse hovers over the record in question.  Another is to place a checkbox
next to each record in the list, then triggering a <submit> based on one
additional keyboard or mouse-click event.

Any opinions?
