I wrote such an application for the company I used to work for. It was set up so that any company that wanted to use it could customize it to their liking. You can see a demo at: http://cso.cgintl.com To log in for a demo, use "demo" (no quotes) for the user id and password. The signin is linked to a customer number (or a list of customers, for example if you have multiple customers that are subaccounts of a main account). There are about 20 or so different companies using this. It's worked flawlessly for quite some time. Aaron Bartell took over that project when I left.
