Using

view.doCommand("set insertMode on");
I had added some logging in also to verify the mode was being changed, and
it is. I also turn off the prefixArea in there and that sticks, but
insertMode does not. I'm guessing they turn it off after initialization
and before display so it acts more like SEU.

Is there a way to force Insert mode on when a member is loaded? I can't
find a config for it.

I even tried setting it within my LPEX plugin, but the member still loads
up in Replace mode. RDi must reset it sometime after if calls the user
profile class and before it displays the member.

Can you post the code in your userProfile class?
I would have expected that

view.doAction(view.actionId("toggleInsert"));

would do the trick.

If not, is this failing on a specific source type? Maybe the parser is
over-writing the insert/replace mode...
