Bingo! There was a couple of them and one of them had a fixed-format C Spec that was out of position (character deletes that moved the Factor 1 and Op Code out of alignment) and as I inserted the two spaces, the other yellow triangles went away and the Outline appeared like magic.

By chance do you have little tiny yellow markers on the right hand-side of your LPEX editor window?
If so, hover your mouse over to see the error....
It could be that you are missing a small syntax error, that prevent the load of the outline view...
