I'm not sure if I interpreted the original message correctly. The new file fields are in the existing file object, right? The outline is picking up file information based on the file object, not the file source.

If the file object has been created with the new fields, my suggestion is to run a verify with prompt to refresh the cache. (Maybe this is the same as performing the clear cache you did, but I'm not certain.)

Yes, Live Outline is active.
When I click the "refresh" link, the outline does refresh, but the file definition in the outline view does not reflect the current file.
It is not a library list issue either, as this is a new file that only exists in one library that is in the library list of the connection I am working in.

Does the outline refresh if you click the refresh icon (the arrows
curving toward each other)?

Is Live Outline turned on?
Windows > Preferences, search for RPG, it's in Remote Systems LPEX
Editor > IBM i Parsers > ILE RPG. In the bottom section, there's a
check box called 'Update the Outline view when text is changed in the
editor'.
