This sometimes happened to me when there was an error in the source. I can't remember what particular kind(s) of error caused it, but for some reason I am thinking about bad H-spec options and incorrect d-specs. Try the verify option before trying an outline refresh - the verify (IIRC) would bring up an error or two that indicates what might be causing the outline's refresh to fail.
