We haven't been able to determine the root cause, but the following is known:

-- IBM believes it may be a specific Op Code or series of Op Codes causing the issue. Something is returning a Null event during the Verify or Outline process.
-- It happens (in my case) with a /COPY book. I've done some whittling down of the code to isolate it, but haven't cornered it. The /COPY book causing it has 28,000 + lines of code, not by our design (by vendor) which is why it's not easy to zero in on.

That's about all I can offer at this time.
