No, using the mouse that came with the PC, DELL. I just tested your theory
about clicking the wheel while scrolling. It didn't cause the application
to close. Someone from IBM contacted me and said that it appears that
there's a graphic resource problem when it happens.

Some versions of Logitech Mouseware do not play nice with other IBM
products when you use the mouse wheel - especially Code/400 and iSeries
Access Setup.

Regardless of the mouse manufacturer, try updating your drivers with the
latest version.

Also, make sure you have not mapped 'Application Close' to the scroll
wheel click function (if your scroll can click like a middle button, and
if your mouse software allows mapping of keyboard functions to mouse
actions)....
