Have you looked at system manager to see if the user accidentally used the
wrong option on the menu.  Check the tasks that the user used on the day of
the transaction to confirm which options were used.

Hello List,

           Raw parts from customers are received (3/INP) into a stockroom
called 2B. In the stock movements inquiry (4/INE) the Movement Type is
normally recorded as:R- Consignment R. The Reason is listed as R- Receipt.
For this particular transaction the Movement Type is listed as "A -
Adjustment" the 'Reason' as listed as R- Receipt. The receipts are only
made through 3/INP. We have hard coded the Reason Code for receipts to be
"R" so there is no need for the user to enter a Reason code. Besides, after
having looked at his entry the user thought that he may have made a
mistake, so he reversed the adjustment and re-received the part. The system
however again received it as Adjustment. This mis-movement happened only
once. We have not seen it again. Any thoughts
