Did you check whether your library list is set correctly?
May be it tries to read from a wrong file.

Looking at what you sent - its tough to determine what fields have what
value Can you put the data into a spreadsheet and copy and paste that into
the e-mail No need to put the entire record - just the values of the keys

How does it fail
