Also an unsuccesful write - for example when a constraint prevented the
record to be written - will result in the loss of the provided identity
value. It will not be used for the next write.

The default *is* no cache but if you are deleting records and
inserting new records then you will definitely see a gap. The database
remembers the last identity it used -- not the last identity in the table.
