Jon Paris has a really good presentation on how to avoid that issue.

My experience is that these collisions, when they happen in an organization, usually happen only in one or two places.  Fixing those one or two places gets rid of 90%+ of the collisions.

The chance of X happening is closer to 100% than I used to think it was.
At my company, it happens often. Our customer service folks use two 5250-based ACS sessions. Both sessions using the same application. They'll often pull up a record on one session (drilling down to update it), immediately toggle over to the other session to fetch related information (and be on the phone with the customer at the same time), not realizing they've locked the record on the other session. Then they call us wondering why the computer tore up! Such is real life.
