The comma is simple enough, if inside a string field, only look for a
ending double quote.

As far as the embedded double quote, when you're inside the string and you
find a possible ending double, look at the next character. If it's a
double quote also, keep going..

I'd then set a flag so you know that there's an "" that you want to
transform to " in the final string.
