Seems to me that that sentence identifies the qualities that would bring it into a "standards" discussion.

1. The %eof/%found usually ("standard") refers to a particular I/O operation on a particular file.

2. ...unless ("exception") you could have just read one of a number of files.

IOW, have the "standard" reflect whatever alternative is 'standard' rather than an 'exception'. If it refers to a particular file /usually/, then make it the "standard" to include the file name.

You can get an additional benefit during maintenance and perhaps at other times due to explicit references. While it might appear cumbersome at first for some, almost any standard will become easy to interpret after it's used a number of times.
