Does anyone know if it be possible for an operating system's memory
manager to do the same sorts of optimizations?

On Wintel I'd say no. On IBM i... :) We've got this bizarre hibrid on
IBM i of 128-bit "pointers" sitting on top of 64-bit hardware. I suppose
it would be _possible_ for the memory manager to use these pointers as a
level of indirection allowing it to basically build a cross-reference of
128-bit "virtual" addresses to 64-bit "real" addresses. However, I think
that somewhat flies in the face of TIMI and the single-level store.
