Yes it does Brad. There are many applications in which you shouldn't
check for existence beforehand. The overhead of checking for existence
beforehand can be a killer on applications where the possibility of a
duplicate are minimal at best.

This can boil down to the same argument of Monitor versus Test. However,
here we're talking disk access and I/O is always more expensive. Did you
look at any of the previous examples provided? Like the manual journal?
How about a one shot file conversion like from an older version of vendor
software to a newer version? Hint: There are duplicate keys because of
the vendors resistance to any use of unique keys in the file definition or
any other constraints and some updates done outside of their software to
merge databases.
