RE: Form-Type entry invalid or out of sequence error.....

Hi

As the program is RPG/400 you cannot have a blank line in there at line
22.00, that will be giving you the Form Type error.  Remove that line or put
an "*" in column 7.

The other error I think is that you are calling subroutine CVTDAT (line
9.00) but the subroutine is called VTDAT (line 16.00)
