Is the file SHARE(*YES) or it it overridden to SHARE(*YES) anywhere? If so,
this is probably a factor. You can get this error if a file which is
SHARE(*YES) is opened for input first then another program attempts to open
it for update but shares the pre-existing ODP.

These things can be sporadic in the way you describe because the order of
events is significant and that can be unpredictable (e.g. it might depend on
the sequence in which a user selects options from a menu).

SHARE(*YES) is a Bad Thing IMHO. Light blue touch paper and retire
immediately...
